As Outreach Worker you serve as an organizer for the B.J. Smith Recreational Center in Chamberlain Hills, as well as other community centers as they are developed.
You will provide client-centered care, working in partnership with clients. You will be expected to facilitate group programming, manage referrals and triage processes, and offer individual support to clients currently attending the service. Your skills include the ability to make behavioral observations, mental status assessments, risk assessments, monitoring of clients’ responses to care interventions and documentation and ongoing communication with other team members and primary care partners. In addition you must be capable of providing supervision to assistant workers and contributing to the development of best practices unique to mental health and concurrent disorder treatment. You will be expected to contribute to program development, research and educational activities of the service.
CORE RESPONSIBILITIES
The main responsibility is daily attendance at our community centers, reaching out and connecting with citizens in various communities around the city as well as participants in Government programs. You will act as a liaison between the communities and the Government at a ground-level; listening to needs and desires and communicating them effectively to the Department of Health and Human Services.

Community development seeks to empower individuals and groups of people with the skills they need to effect change within their communities. These skills are often created through the formation of social groups working for a common agenda. Community developers must understand both how to work with individuals and how to affect communities' positions within the context of larger social institutions.

1. A highschool diploma is required, and an Associate's Degree or above is desired but not required.
2. A valid San Andreas driver's license, or the ability to utilize an alternative method of transportation when required to attend job-related functions.


1. The ability to communicate in writing to include using correct grammar and spelling and to write legibly, clearly, and concisely as needed to document statements.
2. Excellent organizational and time management skills together with the ability to prioritize and manage significant workload under pressure without supervision.
3. The ability to maintain a calm and professional approach when faced with very challenging clients is essential.
4. Applicant must maintain impartiality and objectivity and adhere to strict confidentiality.
5. Knowledge of administrative and clerical procedures and systems such as managing files and records, and other general office procedures.
6. Strong social skills paired with the volition to work with people of various backgrounds.
